Website and coaching terms
Website and coaching content is educational and does not establish a physical-therapy provider-patient relationship or replace medical evaluation. Performance coaching is not medical care. Stop activity and seek appropriate professional or emergency care for concerning symptoms. Coaching scope, billing, cancellation, and service terms will be presented before enrollment or payment.
Cancellations and missed appointments
Cancel or reschedule at least 24 hours before your appointment. A late cancellation or no-show may be charged the full scheduled visit cost. MOTiV may make exceptions for emergencies, severe illness, dangerous weather or significant circumstances.
Late arrivals
Your appointment ends at its scheduled time even if you arrive late. The remaining session will be shorter, and the full appointment fee still applies. Contact MOTiV as soon as possible if you expect to be late.
What a PT package covers
Packages belong to one named patient, cannot be shared or transferred, and expire one year after purchase. They cover PT follow-up visits only. The initial evaluation, specialty assessments, standalone dry needling, coaching and other separate services are excluded unless a written amendment includes them. MOTiV will disclose any added fees and obtain your agreement beforehand.
Unused or missed package visits
Unused visits are not routinely refundable. Exceptions include billing errors, refunds required by law, MOTiV permanently stopping the purchased care, or the therapist finding that further package care is inappropriate or unnecessary. Any refund uses the exact package rate for visits already completed.
A missed package appointment may result in a separate fee up to $125 or the loss of that package visit.
Monthly coaching plans
App coaching has a one-month term and renews automatically. You may cancel anytime. After cancellation, you will not be billed for the following month.
