What is cash-based physical therapy?
Cash-based physical therapy is physical therapy you pay for directly, at a price the clinic publishes, with no insurance claim in between. "Cash" is shorthand: you can pay by card.

"The other huge benefit is it allows me to be one-on-one. This allows for the best experience from an assessment perspective."
At an insurance-based clinic, your plan can limit how many visits it covers, and your share of the bill may not be clear until the claim is processed. At MOTiV you see the price before you book.
MOTiV is self-pay only. It does not accept Medicare, Medicaid or commercial insurance. You pay for your care directly using the published visit or package prices.
What does a self-pay visit get you at MOTiV?
A one-on-one visit with Storm Wilson, PT, DPT, OCS, a board-certified orthopedic clinical specialist. Storm sees every client, from the first evaluation on, inside a full gym where rehab can turn into training. Self-pay is how MOTiV runs all of its injury rehabilitation.
Your evaluation is about 60 minutes and includes completing intake forms that screen for precautions. Follow-ups generally last 50 to 60 minutes. Storm assesses movement and strength, chooses exercise for your goals and uses hands-on treatment when appropriate. He tests and retests within the visit to check what changed. The MOTiV Performance portal supports home exercise programs and exercise guidance; paid virtual coaching tiers are listed separately. Storm provides rehab after ACL reconstruction and meniscus surgery. If your concern falls outside what he treats, he will refer you elsewhere.
How much does cash-based physical therapy cost at MOTiV?
$150for the initial evaluation
$150 for the initial evaluation. All follow-up visits within one episode of care are$125. A package of 8 follow-ups brings each visit to $106.25, and 12 bring it to $100.
| Option | Price | Per visit | Notes |
|---|---|---|---|
| Initial physical therapy evaluation | $150 | single visit | Your first visit. Paid once. |
| Follow-up visit | $125 | single visit | Pay as you go, one visit at a time. |
| 4 follow-up visits | $450 | $112.50 | Saves $50 against 4 single follow-ups. Valid one year. |
| 8 follow-up visits | $850 | $106.25 | Saves $150 against 8 single follow-ups. Valid one year. |
| 12 follow-up visits | $1,200 | $100 | Saves $300 against 12 single follow-ups. Valid one year. |
What changes the total?
The number of follow-ups. Two people with the same injury can spend very different amounts depending on how often they come in and how much they do between visits. The 12-visit package saves $300 against paying $125 twelve times, but only if you use the visits within the year.
Be Strong clients get 10% off single visits and packages, but not a la carte services, and the discount cannot be combined with other promotions. Testing and assessments are priced separately; the MOTiV pricing page lists every service.
Can I submit a self-pay visit to my insurance?
You can ask your insurance company, but MOTiV does not bill insurance and cannot tell you what your plan will pay back. Whether any of it comes back to you depends on your plan, so check before you count on it.

Questions worth asking your plan before your first visit:
- Does my plan pay anything for out-of-network physical therapy?
- If it does, what paperwork do you need from me and from the clinic?
- Do self-pay visits count toward my deductible or out-of-pocket maximum?
- Does reimbursement need a physician referral or a visit limit?
Write down the answer and the name of the person who gave it. Plans differ, and the answer for a coworker's plan may not hold for yours.
MOTiV provides an itemized superbill on request for clinical services actually delivered. Reimbursement is not assured. For services unsuitable for a clinical superbill, you can receive a receipt. MOTiV accepts HSA and FSA cards, as well as cash and the other payment methods listed above.
How do I know when more self-pay visits are worth it?
Keep paying for visits while each one changes something you can name. When you can train on your own and know what to do if symptoms come back, that is a reasonable point to stop.
Ask Storm at the evaluation how many visits to expect and what would count as done. Then buy the smallest package that covers that estimate. A single follow-up at $125 is the right buy if you are not sure yet.
Storm reports a typical course of 6 to 8 appointments, with your needs and progress guiding the plan. Packages cover PT follow-ups only, not the evaluation, specialty assessments, standalone dry needling or coaching unless a written amendment includes them. Each package belongs to one named patient and cannot be shared or transferred. Read the cancellation and package policies before buying.
